Add a new supervisor
Purpose: Add or change a supervisor or co-supervisor to oversee the thesis and research.
- When to submit: As required.
Note: The name of the supervisor/co-supervisor must be added in the comments box with the preference (primary supervisor or co-supervisor).
Add or modify a thesis title
Purpose: Add/modify a thesis title or a research topic.
- When to submit: As required.
Note: The thesis title must be added to the comments box.
Approval of final version of thesis
Purpose: Document your supervisor's approval of the revisions made to the initial thesis and their approval of the final thesis for submission to the library.
- When to submit:
- Following a Verdict 1 at the oral defence: Within a month of their defence.
- Following a Verdict 2 at the oral defence: One hundred and twenty (120) days after the date of the defence to complete the required revisions and to submit the final version of the thesis.
- Forms to attach:
- A PDF copy of the final thesis attachment.

Major Research Paper Super
Purpose 1: Intended for Epidemiology Master students in the Research Paper stream only.
- When to submit: By the end of the third term.

Purpose 2: Submission of the Research Paper for evaluation and other Research Paper documentation.
- When to submit: Submission deadline of the Research Paper will vary on the term.

Research Paper Guidelines for Epidemiology
Request supervisor approval for supervision
Purpose: Confirm the supervisor and co-supervisor (if applicable) to oversee the thesis and the research paper. (Intended for Epidemiology Master students only)
- When to submit: By the end of the second term.
- Note: The name of the supervisor must be added in the comments box with the preference (primary supervisor or co-supervisor).
Exception 10-hour-rule
Purpose: Request permission to work more than 10 hours a week.
- When to submit: As required.
- Form to attach: Request for an Exception to the 10-Hour Rule (PDF).
Extension of the time limit
Purpose: Extend the time limit to complete the program.
- When to submit: At least one month before the student’s time limit.
- Student time limit per level:
- Master: 4 years
- Diploma: 3 years
- Doctorate: 6 years
- Fast-track: 7 years

List of examiners for evaluation of the thesis
Purpose: Submit list of examiners for thesis evaluation before the defence, following their acceptance to evaluate.
For MSc candidates
- When to submit: 1-2 weeks prior to submitting the thesis
- Form to attach: List of Examiners for the Evaluation of the Thesis (PDF)
For PhD candidates
- When to submit: One month prior to submitting the thesis.
- Forms to attach:

- An abstract of the thesis
- The student's CV
- The external examiner’s CV
Plan of study
Purpose 1: Submission of the request to enroll to the Practicum (PHR6910). (For PHR students)
- When to submit: Forms must be submitted one month prior to the beginning of the term wanted.
- Form to attach: Practicum Agreement (PDF)
Purpose 2: Submission of the Thesis Proposal. (For Epidemiology MSc/PhD students only)
- When to submit: Forms must be submitted one month prior to the beginning of the term wanted.

Purpose 3: Submission of the Individual Study Plan (ISP). (For Epidemiology Doctorate students only)
- When to submit: Before initial enrolment in the program.

Progress report
Purpose: Submit an Annual Research Progress Report.
- When to submit: Submission will vary depending on the term and program of study.

TAC Report (or Thesis Advisory Committee Report)
Purpose 1: Submit a Report of the Thesis Advisory Committee or the Nomination of the Thesis Advisory Committee Members.
- When to submit: Submission will vary depending on the term and program of study. For more information, please consult your milestones in your Student Center.
- Forms to attach:
Purpose 2: Submission of the permission to write form. (For BCH/MIC/CMM/NSC)
- When to submit: : A student should write and prepare to submit a thesis only after he/she has received approval from the thesis advisory committee (TAC).
- Form to attach: Permission to begin Full-Time thesis writing (PDF)
Submission of thesis for evaluation
Purpose: Submit the thesis for evaluation.
Note: this request is submitted simultaneously with the Service Request ‘List of examiners for evaluation of the thesis’.
- When to submit:
- Following the first evaluation of the thesis, if the candidate has decided not to proceed to defence the revised thesis must be resubmitted within 120 days of this decision.
- Following a Verdict 3 at the first thesis defence, the student must submit the revised thesis for evaluation within three (3) consecutive terms after the initial defence.
- Forms to attach:
- A pdf. copy of the thesis.
- If submitted a revised version of the thesis, candidates are encouraged to include a separate document listing the changes made and identifying the corresponding page numbers in the original thesis as well as in the revised version. Should a candidate decide not to address some of the concerns of the examiners, it is recommended to give the reason for this decision.
Oral Defence
Purpose: To confirm that the candidate would like to proceed to the defence.
- When to submit: Within 5 days of receiving the email with the thesis examiners feedback.
- Forms to attach: Confirmation must be added in the comment box, i.e. "I wish to proceed to defence", or " I have decided to amend my thesis and make revisions, and therefore not to proceed to defence at this time."
Submission of contract with TTBE
Purpose: A student may request an embargo on public access to the thesis in certain circumstances, such as where publication of the thesis carries confidential or proprietary information.
- When to submit: At any time following submission of the thesis for evaluation. As the embargo period applies to the final version of the thesis, it is preferred to include the completed Embargo request for with the Approval of final version of the thesis following the oral defence (Please allow a few additional days to obtain approval from the department Chair)

Leave of absence
Purpose: A student may apply for temporary leave from his or her studies on medical or compassionate grounds.
- When to submit: Usually due by the third week of the applicable term. Please consult the University’s Sessional Dates page for more exact dates.
- Forms to attach:
- Request for Leave of Absence (PDF)
- If applicable: Additional supporting documentation (ex: medical certificate)
Deferred mark
Purpose: To obtain an extension for the submission of a final grade.
- When to submit: Must be submitted within 5 working days of the missed assignment or exam.

- If applicable: Additional documentation (ex: medical certificate)
Modification or cancellation of registration
Purpose:
- Add or remove a course and change of the Academic Program/Plan
- Enroll in the directed studies course.
- Voluntarily withdraw from your program of study.
When to submit: Consult the University's Sessional Dates page for the relevant deadline and the course and exam timetables for more information on the courses offered during the desired term.
