New:
As of July 1st, 2020, CMPA will only maintain a calendar billing cycle that runs from January to December.- On July 1st, 2020, all trainees will receive an invoice for CMPA membership fees from July 1st to December 31st, 2020.
- On January 1st, 2021, trainees will receive an annual invoice for fees from January 1st to December 31st, 2021.
Renewal is automatic; you do not need to provide renewal information unless there is a change in your contact information or payment method.

CMPA fee reimbursement
Eligibility criteria for reimbursement are set by each province/territory. In Ontario, please visit the MOHLTC website below. Renewal of your CPSO license is done online.Once you have electronically submitted your LOA to the PGME office, it will automatically be forwarded to the CPSO. CPSO will then send you a renewal package by e-mail. The package will include deadline and renewal instructions.

File status: Please ensure to view your online profile as well as your Physiciansapply online account regularly throughout the re-enrollment process.*Independent Practice License (IPL): Application deadline is mid-May, 2020.
*CLINICAL FELLOWS: Any resident completing a residency program and being subsequently appointed as a Clinical Fellow must either renew his/her Postgraduate Education license or apply for an Independent Certificate of Registration if intending to bill OHIP for services while pursuing the Fellowship. No trainee will be permitted to bill OHIP with an Educational license.

Electronic Letter of Appointment (eLOA)
Submit electronically. You will receive a message confirming “Successful submission”.
PGME will forward your eLOA to CPSO on your behalf; CPSO will not accept it unless it comes from PGME.
Starting after July 1st (off-cycle)?
- The LOA covers only one academic year at a time (July 1st to June 30th). If you are starting after July 1st e.g. August 1st, the remainder of your training will be reflected on your LOA for the following academic year. An LOA will never go beyond June 30th.
Error(s) on eLOA:
- If the address on the eLOA has changed, please submit your eLOA online and inform your program administrator of the update.
To obtain a copy of eLOA: Refer to “forms” in your checklist via the Enrollment Website.

Emergency Preparedness
In the event of an emergency, a notification will automatically be sent to your uOttawa email. The Emergency Notification System (ENS) ensures you can be reached efficiently and effectively through a variety of methods. If you wish to be notified through your smart phone or tablet, please sign up via Protection Services.

Immunization & N95 Mask Fit Testing
The Risk Management Nurse Advisor will be contacting you directly regarding your immunization requirements, including timelines.
This includes annual TB testing, Diphtheria-Tetanus immunization every 10 years, and N95 Mask Fit Testing every 2 years.
Important:There are no religious or philosophical accommodations for immunization and N95 Mask Fit Testing requirements.
Failure to comply will result in a clinical suspension without pay.
Please send your results to the Risk Management Nurse Advisor, and not the hospital’s occupational health.

Leave of Absence (Maternity Leave, Sick Leave, etc.)
Mandatory requirements apply during a leave of absence:
- eLOA
- renewal of CPSO
- registration fee payment.
Prior to your return, it is your responsibility to ensure that you are fully compliant with:
- renewal of CMPA
- immunization, N95 Mask Fit Testing & VSS
- e-learning modules
If you plan on taking vacation at the end of your leave, please inform the PGME office and your Program Administrator.

Registration Fee (uOttawa)
Cost: $805 annual fee payable to the University of Ottawa
You will receive an invoice and communications from the Financial Resources Office on how to pay your Registration fee, and you will require your University of Ottawa student/account number in order to do so. Your student number can be found on your enrollment notice. Please note that you cannot start training until your fees have been completely paid.
Visa Trainees: If you are sponsored by the Saudi Arabian Cultural Bureau, your registration fees will be paid directly by the sponsor.

VSS or Self-Declaration
Trainees must complete the Vulnerable Sector Search (VSS) Self-Declaration form online available on the Trainee Checklist on the Enrollment Website.
The Office of Risk Mangement is coordinating this effort, please watch for communications and instructions from them.
Trainees will also be required to complete a HSPnet consent form for the purpose of storing information on our database. Contact the Risk Management Nurse Advisor for further information.

At the end of each module (and in order to receive credit) you must submit the statement of completion by completing.
The following modules must be completed annually:
- Module 1: WHMIS - To be renewed in April 2021
- Module 2: Privacy and Information Security
- Module 3: Emergency codes
- Module 4: Tuberculosis Management

The University of Ottawa’s online student services can be found on the uoZone student portal. Access it for financial statements, tax forms (T2202), etc.
Username: uoAccess ID is the short name before your @uOttawa.ca e-mail address (e.g.: jsmit000).
